Patient Navigator Lead

Under the direction of the Program Manager, the Program Lead provides support and coordination of programmatic activities in addition to delivering direct services.  This position is responsible for supporting program operations, leading projects, and providing an additional level of support for Patient Navigators

Engage potential patients and schedule new patient appointments

Prepare and assign potential patient list to Patient Navigation team for outreach

Assist with screening and enrolling new ECM clients

Provide care coordination for client 

Track program activities/outcomes, and prepare biannual reports

Document and maintain records per appropriate standards and regulations

Attend and participate in relevant community activities and meetings

Supervise, evaluate, and provide performance feedback to program staff on a regular basis

Provide and ensure continuing training/education for staff for on-going development

Prepare Patient Navigator’s weekly schedule

Conducts biweekly team meetings

Complete other duties and related projects as assigned

High School Diploma or Equivalent

Three or more years of patient navigation or case management experience

Two or more years’ experience working in a health care setting

Proficient with Microsoft Office applications

Experience working with Electronic Health Records

This position requires frequent driving. A valid California driver license, current personal auto insurance, and an MVR sufficient to maintain insurability under agency auto liability policies.

Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.

Ability to communicate well with people of diverse cultural professional and experiential backgrounds and

Language fluency verbal and written, in one of the following languages: English, Spanish, Vietnamese, Mandarin, Tagalog, Farsi highly desired

Patient Navigation Certification highly desired
